- 33-11-4:Basic - Missing drain plug at dumpster. No drain plug. Leaky garbage and cardboard use.
- 36-02-5:Basic - Unsealed concrete floor in public restroom, and entire establishment.
- 42-01-4:Basic - Wet mop not stored in a manner to allow the mop to dry. Observed mop not stored to dry properly.
- 29-44-4:High Priority - Air gap between water supply inlet and flood rim of equipment/sink is less than one inch. Observed drain pipe under prep sink set all the way down in the floor, no air gap above floor surface.
- 35A-02-6:High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 1 live fly around drain below prep sink in kitchen
