- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed a black mold-like substance on the top interior of the ice machine.
- 24-07-4:Basic - Cleaned and sanitized equipment or utensils not properly stored. Observed four cutting boards being stored on the floor in between the prep table and reach in cooler.
- 14-09-4:Basic - Cutting board has cut marks and is no longer cleanable. Observed several cutting board in the kitchen area with many cut marks throughout.
- 12B-07-4:Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed one employee drinks being stored on the shelf above the three compartment sink. Observed one employee drink on the shelf near the handwashing sink on the cook line.
- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed storage rack in the kitchen area near the cook line soiled with old food debris. Observed the hood system with a build up of grease. Observed the storage shelves with build up debris in the kitchen are near the cook line. Observed gaskets being soiled with food debris in the reach in coolers in the kitchen area. Observed grease build up on the pipes behind the cooking equipment on the cook line.
