- 14-11-5:Basic - Equipment in poor repair. Observed gaskets torn on walk-in freezer, walk-in cooler , kitchen reach in cooler, operator has attempted to repair gaskets with screws. Gaskets no longer seal doors when closed.
- 14-69-4:Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed build up of ice inside walk-in freezer.
- 41-07-4:High Priority - Container of medicine improperly stored. Observed over the counter medication, first aid supplies, and over the counter supplements stored above establishment food in dry storage. Discussed with person in charge.
- 12A-18-4:High Priority - Employee dried hands on clothes/apron/soiled towel after washing. Observed kitchen employee wash hands when inspector entered kitchen and then dried hands on wet cloth. Discussed with person in charge and employee. Employee re washed hands and dried with paper towels **Corrective Action Taken**

Observed one kitchen employee hired over 60 days per person in charge missing proof of training.

- 09-01-4:High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed operator stacking sushi in reach in cooler without gloves or utensils. Discussed bare hand contact or AOPs with operator. Operator washed hands and placed gloves on. **Corrective Action Taken**
- 01D-01-5:High Priority - Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Observed invoice from Great Wall Seafood VA, LLC does not have parasite destruction information for Escolar. Establishment does not have other parasite destruction from this distributor. Instructed operator to acquire parasite destruction before serving sushi with Escolar. Operator was not willing to call distributor during inspection after being requested multiple times.
