- 06-09-1:Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. 4 packages of raw tuna in walk in cooler fully thawed out still inside packages. **Repeat Violation**
- 13-07-4:Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Employees with several bracelets preparing food in prep area in kitchen.
- 36-73-4:Basic - Floor soiled/has accumulation of debris. All floors under equipment in kitchen, ware washing, walk in cooler, walk in freezer, and bar area. Also walls behind equipment on cook line have a build up of debris.
