- 14-11-5:Basic - Equipment in poor repair. Observed on the interior door to the walk in cooler has a hole on the bottom and you can see the insulation on the inside of the door.
- 08B-12-5:Basic - Stored food not covered. Observed in walk in cooler multiple containers of cut broccoli and cabbage, stored on crates, stacked on top of each other and not covered.

Observed a pan of raw chicken stacked on top of a pan of raw pork on shelf in walk in cooler. Observed a pan of raw chicken on shelf over shell eggs in walk in cooler. Observed stir fried rice in warmer on table across from woks at 100F. Operator stated that they reheat it later when needed for an order.
