- 36-34-5:Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles has terrible dust accumulation also the AC vent above three compartment sink.
- 22-08-4:Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed old food splashed on side panels of microwave oven.
- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed greasy hood filters underneath fire suppression above fryers, also back area of both fryers spoke to Manager concerning notify the fire service cleans the hoods.
- 14-20-4:Basic - Ripped/worn tin foil used as shelf cover. Observed aluminum foil ripped used on shelves next to grill.(cook line)
- 08B-12-5:Basic - Stored food not covered. Observed chicken strips not covered in the reach in freezer.

- 36-06-4:Basic - Cardboard used on floor as anti-slip measure not replaced every day or when heavily soiled, whichever comes first. Observed greasy cardboard on the floor in front of the fryer.
- 14-71-4:Basic - Duct tape used to repair nonfood-contact surface. Observed door handle has tape on the door handle reach in cooler used near the fryer.
- 16-46-4:Basic - Old labels stuck to food containers after cleaning. Observed old labels stuck on plastic containers stored on shelves.
- 22-16-4:Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed old food debris spilled on the bottom of reach in freezer.
