Floors under drive thru soda machine, drive thru register counter, under and around bag in box soda rack, in walk in freezer and under all flip top reach in coolers in kitchen have debris and/or soiled. Multiple vents and surrounding ceiling tiles on make line and vents above reach in cooler across from back prep area are dusty. Fan guards in small walk in cooler have debris. **Repeat Violation**
  • 02D-03-4:Basic - Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. Packaged cookies at front counter for customer self-service list ingredients and corporate address. Per manager, cookies are packaged on site. No date of packaging observed. **Repeat Violation**
  • 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Under plating around soda nozzles on lobby soda machine are soiled. Counter under lobby soda machine is soiled.
  • 16-32-5:Intermediate - Incorrect/ineffective/inaccurate chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine. Manager provided chlorine test strips. Strips provided will measure chlorine in the dishmachine to only 25ppm chlorine according to color concentration chart provided. Provided test strips were not past expiration date. Inspector's chlorine strips were used to verify proper concentration and tested twice at 75ppm chlorine.
