Observed employee touching the basket handle of the fryer, and then began to prepare the orders, touching clean container and paper bags without changing gloves. Discussed and coached employee in proper methods. Employee washed hands.
- 29-34-4:High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breaker valve at hose bib, located at mop sink.

- 14-69-4:Basic - Ice buildup in reach-in freezer and/or walk-in freezer. At the time of the inspection observed ice in reach in freezer located near the cool lime
- 31B-04-4:Basic - No handwashing sign provided at a hand sink used by food employees. At the time of the inspection observed no hand wash sign in the restroom.
- 33-16-4:Basic - Open dumpster lid.
- 03A-02-5:High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At the time of the inspection observed raw chicken (45F - Cold holding) over ice at cook line as per manager for approximately 20 minutes. Advised manager to add ice to surrounding container. Observed Cooked fries (67F - Cold holding) at room temperature at cook line, as per manager for approximately 10 minutes. Employee placed item inside fryer to reheat to 165F. **Corrective Action Taken**
