- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed at drive thru ice bin.
- 22-13-5:Basic - Coffee dispenser not cleaned at least once every 24 hours. Observed coffee machine dispenser soiled with black mold like substance. Employee began to clean during inspection. **Corrective Action Taken**
- 50-09-4:Basic - Current Hotel and Restaurant license not displayed. Observed license dated 10/1/2024 posted at establishment office. **Repeat Violation**
- 14-69-4:Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed at cook line overhead reach in freezer, ice build up.
- 10-12-5:Basic - In-use ice scoop stored on soiled surface between uses. Observed at drive through ice bin, ice scoop stored in soiled holder with mold like substance. Manager removed to clean. **Corrective Action Taken**
- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed at several reach in coolers and freezers gaskets soiled with build up of black mold like substance. **Repeat Violation**
- 16-46-4:Basic - Old labels stuck to food containers after cleaning. Observed on empty cleaned blue containers in walk in cooler. Manager removed. **Corrected On-Site**
- 29-49-6:Basic - Standing water in bottom of reach-in-cooler. Observed at prep reach in cooler next to ice tea dispenser.
- 03D-07-5:High Priority - Time/temperature control for safety food removed from cold holding for necessary preparation rose above 41 degrees Fahrenheit during the preparation process and was not cooled back down to 41 degrees Fahrenheit within 4 hours. Observed at prep cooler 16 containers of lettuce (50F - Cooling) and 2 containers sliced tomatoes (51F - Cooling). As per label was prepped 3.5 hours ago. Manager removed from cooler and placed into walk in cooler for rapid cooling. **Corrective Action Taken**
- 22-02-4:Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed opening of fry dispenser soiled with black mold like substance/ slime. Observed drive thru creamer dispenser soiled.
- 31A-11-4:Intermediate - Handwash sink used for purposes other than handwashing. Observed employee use hand wash sink an end of cook line as dump sink for ice.
- 31B-06-4:Intermediate - Soap dispenser at handwash sink not working/unable to dispense soap. Observed at women's bathroom single stall.
