- 36-34-5:Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed all throughout the establishment.
- 08B-38-4:Basic - Food stored on floor. Observed oil containers stored in the floor at kitchen area.
- 35B-04-4:Basic - Outer openings of establishment cannot be properly sealed when not in operation. Observed rear door exit with large gaps.
- 02C-06-5:Intermediate - Operator is not properly tracking/marking the number of days opened commercially processed, ready-to-eat, time/temperature control for safety food was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Observed several bags of beef and other food items not in original packaging and not dates, as per owner from two days ago.
