Per employee establishment dumps sink water in grass behind dumpster of nearby liquor store and operator dumps waste water at private residence. Nearby liquor store does not have a mop sink or approved wastewater disposal location.

Person in charge stated food items were placed into steam table at time of open, which was 2 hours prior. Person in charge moved food items to flat top grill to reheat to 165F minimum temperature. Observed house made cheese sauce using cream with no date marking. Person in charge stated cheese sauce was made two days prior.
