Observed; deck of playing cards person in charge stated belong to employee stored on microwave across from three compartment sink. Employee cell phone on prep counter next to single service items at front counter. **Repeat Violation**
- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled good filters above cook station. Soiled door frames on reach in cooler across from cook station. **Repeat Violation**
- 12A-18-4:High Priority - Employee dried hands on clothes/apron/soiled towel after washing. Observed employee wash hands in hand wash sink then dry hands on an in-use wiping cloth found on cutting board on flip top cooler across from hand wash sink.
- 09-01-4:High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Observed employee on cook line toast bread on flat top grill then remove bread from grill with bare hands to prepare sandwich with other ready to eat foods. Inspector coached for proper avoidance of unnecessary hand contact.

Observed box of opened raw beef patties over bag of fully cooked chicken tenders removed from their original package in freezer in common area.
