- 35A-03-4:Basic - Dead roaches on premises. Observed approximately 7 dead roaches in plastic pan on floor to right of cookline.
- 14-71-4:Basic - Duct tape used to repair nonfood-contact surface. Tape observed on several kabob skewers.
- 25-32-4:Basic - Reuse of single-service or single-use articles. Observed empty milk gallon containers reused to store and serve water.
- 41-07-4:High Priority - Container of medicine improperly stored. Medicine stored in refrigerator with food products, burn spray stored on shelf with spices in kitchen.
- 08A-04-5:High Priority - Raw animal food stored over or with unwashed produce. Observed raw shell eggs stored over bag of lemons in white reach in cooler in kitchen. In triple door silver cooler, raw chicken skewers stored over pickled turnip containers. One employee working less than 60 days with no proof of knowledge of reporting responsibility. Provided form to operator. **Corrective Action Taken**

- 02C-02-5:Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Hummus made two days ago per person in charge with no date mark. Also beef kibbeh made in house, frozen, then thawed not marked with date made.
- 45-04-4:Use of cooking equipment producing grease laden vapors/smoke with no hood suppression system installed. Notified Fire AHJ. For reporting purposes only. Establishment has two table top fryers in use in kitchen, no hood over units.
