- 35A-03-4:Basic - Dead roaches on premises. Observed 2 dead roaches on the floor behind a file cabinet at the office space located in the kitchen. Operator removed cabinet and cleaned the floor removing the two roaches on site. **Corrective Action Taken**
- 12B-02-4:Basic - Employee eating in a food preparation or other restricted area. Observed employee eating bread at preparation area.
- 08B-49-4:Basic - Employee personal food not properly identified and segregated from food to be served to the public. Observed in the double door glass cooler several employees water bottles not ID or properly identified. Reminder manager that employees personal food and beverages have to be identified from the food serviced to the public.
