  • 35A-03-4:Basic - Dead roaches on premises. Observed one dead roach inside the hand wash sink located at storage/prep area. Observed one dead roach on the floor by the reach in freezer located by the prep sink at preparation area, observed one dead on the floor under the three compartment sink, observed one dead roach behind the reach in freezer located at the preparation.
  • 08B-47-4:Basic - Food not stored at least 6 inches off of the floor. Observed a cardboard box with oranges stored on the floor at front counter.
  • 21-04-4:Basic - In-use wet wiping cloth/towel used under cutting board.
