- 16-44-4:Basic - Dishware/utensils not soaked prior to being placed in three-compartment sink/dishmachine and results in soiled dishware/utensils after washing occurs. Observed: green guillotine slicer still soiled after wash procedure. These were placed in hot soapy soak water, and the staff started the rewash process. **Corrective Action Taken**
- 08B-38-4:Basic - Food stored on floor. Observed: raw onions in a bowl and bulk oil stored on the kitchen floor. ALSO, bulk food stored on the floor in the walk-in freezer.
- 11-27-4:Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Observed: lack of a plan. One was emailed.

- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed: the grey ice activator at the soda machine is soiled.
- 09-01-4:High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed: vegetables added to a plate, no gloves.
