- 08B-49-4:Basic - Employee personal food not properly identified and segregated from food to be served to the public. Observed: multiple items in the reach-in cooler that were described as food for employees only. These were moved to the bottom shelf. Please mark EMP. **Corrective Action Taken**
- 40-06-5:Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed an employee cell phone stored over the front line make table with guest food.
- 16-13-5:Basic - Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed: the sanitizer sink was set up in the middle. All sinks were dumped and reset. Corrected On-Site
- 51-13-4:Basic - No Heimlich maneuver/choking sign posted. Observed: lack of a visible choking poster. One was emailed.
- 03A-02-5:High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: the glass front reach-in cooler on the front line had yogurt, 46°. This was moved to the colder make table. **Corrective Action Taken**
- 41-18-4:High Priority - Warewashing sanitizing solution exceeding the maximum concentration allowed. Observed: the 3 compartment sink chlorine sanitizer was indicated over 400 ppm. This was dumped when resetting the order, and reduced to 100 ppm. Corrected On-Site
- 02C-03-5:Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed milk open prior to today, no date. A date was added. Corrected On-Site
- 12A-03-4:Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed: manager washed hands in the side dump sink.

- 31B-02-4:Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed the common area men,s restroom laxks paper towels.
- 45-05-4:Portable fire extinguisher missing from its designated location. For reporting purposes only. Observed: the red fire extinguisher is not at a signed designated location. There is one under the hot water heater.
