- 14-11-5:Basic - Equipment in poor repair. Door on cold display cooler repaired with duct tape. Cutting boards on cook line and server alley reach in coolers with deep cut marks. Reach in freezer on cooks line the interior left door has exposed insulation and exposed insulation on the outside of right door due to handle being broken.
- 36-73-4:Basic - Floor soiled/has accumulation of debris. Under cooking equipment on cooks line.
