- 32-07-4:Basic - Bathroom facility not clean. Observed 1 toilet in the women's restroom is soiled.
- 14-69-4:Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed: a stalactite and stalagmite formed in the walk-in freezer.
- 05-09-4:Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed: cold hold units lacking an internal Ambient air temperature thermometer. These were then being put into each unit. **Corrective Action Taken**
- 31B-04-4:Basic - No handwashing sign provided at a hand sink used by food employees. Observed all hand wash sink(including restrooms) lack a sign.
- 41-10-4:High Priority - Toxic substance/chemical improperly stored. Observed: gallon bleach stored over food on the dry goods shelf by the office. This was moved to cleaning. **Corrected On-Site**
