- 12A-07-5:High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee on front serving line wearing gloves had cell phone in hand typed on the cell phone, put cell phone away, removed gloves, did not wash hands, then put on new gloves, then handled serving utensils to complete and order.
