- 08B-38-4:Basic - Food stored on floor. Bucket of thawing chicken in water on floor of mop sink.
- 06-01-5:Basic - Time/temperature control for safety food thawed in an improper manner. Observed cooked beef and chicken thawing in standing water in prep sink, in bucket in mop sink.
- 12A-02-4:High Priority - Employee handled money from customer and then picked up plated food, served food, or prepared a beverage without washing hands. Employee handled money from customer and then scooped tortilla chips, no hand wash. Spoke with manager regarding proper hand wash steps.

Observed Sanitizer Bucket (Quaternary 500+ppm) next to cutting board at front counter used to place customer food orders. Spoke with operator about diluting to proper strength.
