- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin, primarily on the inner deflector.
- 51-11-4:Basic - Carbon dioxide/helium tanks not adequately secured. Six CO2 tanks unsecured by the large ice machine.
- 10-03-4:Basic - In-use utensil used with moist food stored in running water with insufficient velocity to flush food particles away. Dipper well water flow at the ice cream freezer not sufficient velocity.
- 40-01-4:Basic - No suitable facilities provided to store employee clothing and other possessions. Observed personal backpacks, jackets, cell phones, chargers, soiled aprons commingled with food and clean equipment in several areas.
