- 36-03-4:Basic - Cove molding at floor/wall juncture broken/missing. Cove molding has been replaced with spray foam at the juncture between the front and back room.
- 36-11-4:Basic - Floors not maintained smooth and durable. Vinyl flooring in back prep room is torn with large holes, exposing the concrete below.
- 08B-38-4:Basic - Food stored on floor. Numerous cases of condiments, marshmallows and single-service to-go items are being stored on the floor in the front room, most are under the service window counter.
- 41-07-4:High Priority - Container of medicine improperly stored. Bottle of ibuprofen was stored on the dry storage shelving, commingled with ingredients for food for sale to the public. Handed bottle to Operator and explained proper storage. Corrected On-Site

Observed pans of food in the kitchen area out of refrigeration, sitting near the stove. Employee stated food had only been out of refrigeration for 10 minutes. Explained the importance of keeping foods at proper temperature, removing only what is needed to prepare recipes and returning to refrigeration immediately. Employee stated she would return product to the walk-in cooler for rapid cooling-down. Upon exiting the facility to type report 15 minutes later, hot dogs were still on the table. Reminded employee to place and keep under refrigeration. hot dogs (66F - Cold Holding); cooked pasta (61F - Cold Holding); cooked vegetables (63F - Cold Holding); chicken breast (63F - Cold Holding)
