Basic – Bathroom facility in disrepair. Observed women's bathroom missing one sink, second sink not functional . Two operational sinks still available at time of inspection.

Intermediate – Handwash sink not accessible for employee use at all times. Observed employee jacket hung on sink closest to walk in cooler. Operator removed during inspection Corrected On-Site
