Observed 3lbs of reduced oxygen packed fish fully thawed without being removed from their packaging

High Priority – Single-use gloves not changed as needed after changing tasks or when damaged or soiled

Observed an employee clean with a dirty wiping cloth and then pick up clean utensils without changing gloves
