Establishment has no approved Alternative Operating Procedure. Observed employee preparing lobster roll and adding lettuce with bare hands.

High Priority – Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb.Observed hose on splitter in mop sink without vacuum breaker.
