Basic – Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Observed acoustical ceiling tiles in food prep area of kitchen. Repeat Violation

Basic – Ceiling tile missing. Observed missing ceiling tile in rear storage room and missing tile in kitchen next to hood. Manager stated trying to get someone to replace tile by hood. Repeat Violation

Basic – Floors not maintained smooth and durable. Observed portions of kitchen floor with bare concrete.

Basic – Hood filter missing from automatic fire suppression/exhaust system. Observed 1 missing hood filter vane and 2 vanes loose and in disrepair.

Basic – Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed microwave next to hand sink soiled with debris. Repeat Violation

Basic – No handwashing sign provided at a hand sink used by food employees. Observed no hand wash sign at rear kitchen hand wash sink.

Basic – Working containers of food removed from original container not identified by common name.observed squeeze bottles throughout kitchen not labeled.

High Priority – Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breaker installed on Y fitting with attached garden hose. Vacuum breaker must be installed on the leg of the Y adapter that has hose attached. Repeat Violation

Intermediate – Spray bottle containing toxic substance not labeled. Observe unlabeled spray bottles with yellow liquid in them on cart in kitchen. Repeat Violation
