Observed unwashed avocados on cook line with stickers still on them

Manager had employee remove stickers and rewash avocados. Corrected On-Site Repeat Violation

High Priority – Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands

Observed employee pull pants up with gloved hands and proceeded to engage in food preparation

Manager instructed employee to remove gloves and wash hands properly after touching pants.

High Priority – Stop Sale issued on time/temperature control for safety food due to temperature abuse

Observed sautéed onions in a pan next to grill at 62F

Manager stated was on held on time control but General Manager stated onions should be on hot holding

Manager discarded onions.

Observed Raw chicken at end of cook line at 45F

Manager stated chicken was placed in cooler from walk-in cooler this morning

Employee placed ice on top of Raw chicken. Corrective Action Taken
