By ice machine in back kitchen area. Drain pipe for ice machine was disconnected from drain. Manager moved pipe back into place and asked employee to remove water. Corrective Action Taken

Basic – No handwashing sign provided at a hand sink used by food employees

Hand washing sink in men's restroom in dining area. Sign located and posted. Corrected On-Site

Basic – Toilet/urinal not flushing/functioning properly

Toilet in women's restroom in dining area. Manager states repair company has been contacted. Corrective Action Taken

High Priority – Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands

Employee placed soiled rice container down, vent cover on back of reach in cooler fell off onto floor, employee picked up cover and placed cover on and immediately began preparing food for guests without washing hands. Manager coached employee on proper hand washing procedures. Corrective Action Taken

High Priority – Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands

Employee on cook line handling raw chicken bags, placed contents on grill, placed bags in trash container and pushed down with gloved hands and immediately went back to preparing chicken on grill without washing hands. Manager coached employee on proper hand washing procedures. Corrective Action Taken

High Priority – Sewage/wastewater backing up through floor drains

On the cook line, water is backing up under the floor where a hand washing sink once was installed. Manager states that the establishment removed the hand washing sink where water is backing up. Inspector cannot determine the source of backup as the floor is covered in tile/concrete. Warning

At front counter display area, cooked steak 124F, cooked chicken 118F and cooked chicken 120F. Items cooked approximately 15 minutes prior. nIn warmer by cook line, five pans of rice 110F. Rice placed in warmer approximately 2.75 hours prior. All items voluntarily discarded. Corrective Action Taken Repeat Violation Admin Complaint

In back storage area, multiple chemicals on shelf stored above to go cutlery kits. Cutlery moved and stored correctly. Corrected On-Site

High Priority – Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet

Missing at the end of the splitter on mop sink faucet where black hose is attached.

Intermediate – Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed
