Clean stacked pans on storage shelf next to warewashing area unable to properly air dry. Operator separated pans to air dry properly. Corrective Action Taken Repeat Violation

Multiple grab and go containers at front cooler in dining area missing address of establishment that prepared/ packaged foods. Repeat Violation

One flying insect observed in warewashing area.

cut tomatoes (48F – Cold Holding) in top of flip top reach-in cooler across from conveyor oven. Operator stated tomatoes were placed in top of cooler this morning prior to opening. Operator removed sliced tomatoes and placed in walk-in cooler to chill. Retemped at end of inspection at 43F Corrected On-Site

High Priority – Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet

Vacuum breaker missing from non chemical side of splitter. Vacuum breaker installed on incorrect side of splitter. Discussed with operator.

Intermediate – Food-contact surface soiled with food debris, mold-like substance or slime

Cutting boards on reach-in coolers on cook line soiled/ stained. Repeat Violation
