Basic – Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Dust on vents in back kitchen over triple sink and food prep area by walk-in cooler.

Basic – Floor soiled/has accumulation of debris behind cook line equipment. Repeat 8/23/2022. Repeat Violation

Basic – Floor tiles missing and/or in disrepair. Floor damaged in middle of cook line, at walk-in cooler door. Repeat 8/23/2022. Repeat Violation

High Priority – Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, food, or touched unwrapped single-service items without washing hands. Observed employee washing dirty dishes, then wrapped meat for storage in walk-in cooler no handwash. Discussed with employee proper handwashing, employee washed hands before returning to work. High Priority – Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Two way splitter installed, missing one vacuum breaker on right side.

Intermediate – Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Intermediate – Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. In walk-in cooler observed approximately 25# cut tomatoes ambient cooling in large covered plastic container, approximately six inches deep. Provided operator with cooling handout and discussed ambient cooling techniques with operator.

Intermediate – No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Provided operator employee reporting agreement, (HR 5030-309) manager health reporting responsibilities (HR 5030-101)and employee health reporting responsibilities (HR 5030-103], discussed with operator importance of employee heath.
