Basic – Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed mold-like substance inside ice machine. Manager had employee clean during inspection Corrected On-Site

Basic – Covered waste receptacle not provided in women's bathroom. Observed during inspection that women's bathroom didn't have covered trash receptacle

Basic – Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee drink on prep table at start of inspection. Employee moved drink. Corrected On-Site

Basic – Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed during inspection employee phones on soda machine and prep tables.

Basic – No handwashing sign provided at a hand sink used by food employees. Observed during inspection that neither bathroom had employees must wash hands signs.

Basic – Single-service articles not stored inverted or protected from contamination. To go plastic containers not inverted at prep area, corrected , manager inverted containers during inspection. Corrected On-Site Repeat Violation

Basic – Wet wiping cloth not stored in sanitizing solution between uses. Repeat Violation

High Priority – Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee dropped object on floor, picked it up to throw away and back to work without washing hands. Manager spoke with employee. Corrective Action Taken

High Priority – Employee washed hands with cold water. Observed employee wash hands using cold water during inspection. Discussed with manager proper hand washing procedures with hot water to ensure proper hand washing. Corrective Action Taken

High Priority – Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed hose bib with a splitter without vacuum breaker.

Intermediate – Food-contact surface soiled with food debris, mold-like substance or slime. Observed during inspection food debris on juice ,milk and shake dispensers. Manager had employees wipe down all surfaces during inspection. Corrected On-Site
