Observed keys hanging from the front pocket of cook and keys resting on prep table when cook leans on prep table. Observed employee jackets and purses hanging on back of door to dry goods storage area. When the dry goods storage area door is open all of the employee personal items rest on top of the soda syrup dispenser unit.

Basic – Employee with ineffective hair restraint while engaging in food preparation

Observed cooks on cook line with no hair restraint or beard guard

Basic – Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust

Observed mold like substance on the underside of the non food contact surface of the nozzles on the soda and orange juice dispensing machines at server station.

Basic – Old labels stuck to food containers after cleaning

Observed original manufacturer container labels not removed from food containers after cleaning and in use with flour and cocoa, located in dry goods storage area

Basic – Raw fruits/vegetables not washed prior to preparation

Observed produce ( avocado ) located in flip-top make table with produce stickers still on produce.

Basic – Single-service articles improperly stored

Observed exposed and not inverted coffee filters on top of containers where coffee filters are stored at front server station

Basic – Working containers of food removed from original container not identified by common name

Observed working container of food removed ( gluten free flour, cocoa ) from original container not identified by common name.

High Priority – Stop Sale issued due to food not being in a wholesome, sound condition

Observed plate fall and break into pieces on cooking grill. Cook discarded plate and orders of hashbrowns that were on the grill.

High Priority – Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb

Observed hose Bibb outside front entrance of establishment with no vacuum breaker

Intermediate – Handwash sink not accessible for employee use at all times

Observed hand wash sink at the end of the cook line blocked by equipment and trash can
