Basic – Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of ice machine as soiled. Repeat Violation

Basic – Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents in kitchen as soiled with debris, and heavy mold like buildup on pipe on ceiling of walk in cooler.

Basic – Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed open bottle of water on reach in cooler in kitchen.

Basic – Equipment and utensils not properly air-dried – wet nesting.

Basic – Food stored in a prohibited area. Observed cases of alcohol stored in employee restroom.

Basic – In-use ice scoop stored on soiled surface between uses. Observed ice scoop stored directly on top of ice machine.

High Priority – Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed butter portions at front counter at 65F cold holding. Operator moved to another cooler for cooling. Corrective Action Taken

Intermediate – Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Provided Operator with printed copy. Corrected On-Site Repeat Violation

Intermediate – Spray bottle containing toxic substance not labeled. Observed 2 unlabeled spray bottles near ice machine.

Intermediate – Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed no running water at hand wash sink near ice machine in back bakery area.
