Basic – Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Owner called repairman. Corrective Action Taken

Basic – Food stored on floor. Observed boxes and buckets of food stored on the floors in walk in cooler walk in freezer. Observed vegetable oil on floor near the cooks line. Manager removed the oil and placed on top of prep table.

Basic – Interior of oven/microwave has accumulation of black substance/grease/food debris.

Intermediate – Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Emailed form to owner.

Intermediate – Handwash sink used for purposes other than handwashing. Observed spilled liquid debris in hand sinks.

Intermediate – No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths.
