Basic – Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed build up in ice machine located near back door.

Basic – Accumulation of lime scale on the inside of the dishmachine. Observed build up of lime scale in hi temp ware washing machine.

Basic – Bowl or other container with no handle used to dispense food. Observed scooping bowl with no handle left in large bowl of frozen fries in under counter reach-in freezer.

Basic – Cutting board has cut marks and is no longer cleanable. Observed cutting board located in front of roll top reach in cooler with cut marks. Repeat Violation

Basic – Food stored on floor. Observed boxes of fries stored on floor of walk in freezer. Operator removed boxes from floor. Corrected On-Site Repeat Violation

Basic – Old labels stuck to food containers after cleaning. Observed containers stored on shelf with old sticker labels.

Intermediate – Handwash sink not accessible for employee use at all times. Employee hand wash sink located near cook top in kitchen blocked by garbage pail.

Intermediate – Handwash sink used for purposes other than handwashing. Employee hand wash sink near three compartment sink had items stored in it.

Intermediate – No measuring device available for measuring utensil surface temperature when using hot water as sanitizer in a dishmachine. Operator had no test strips or thermometer to measure temperature for dish machine. Food and beverage manager arrived during inspection and was able to locate test strips. Corrected On-Site Repeat Violation

Intermediate – No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Manager did not have files accessible to show employee signatures initially. Observed some completed forms upon arrival of F&B manager. Repeat Violation

Intermediate – Soap dispenser at handwash sink not working/unable to dispense soap. Observed employee hand wash sink in bar area with no soap.
