Basic – Accumulation of black/green mold-like substance in the interior of the ice machine/bin

Ice machine next to drive through windows is soiled at top, rear of machine beneath lift top.

Basic – Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items

Observed employee personal handbag stored on food storage rack. Person in charge moved items to another location. Corrected On-Site

Basic – Floor soiled/has accumulation of debris

Floors behind milkshake machine at deep fryer next to drive through are soiled as well as the walk in freezer and beneath storage rack.

Basic – Ice buildup in reach-in freezer and/or walk-in freezer

Observed ice buildup in walk in freezer on vents.

High Priority – Employee touching ready-to-eat food with their bare hands – food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure

Observed employee make bare hand contact with lettuce and pickles while replenishing the container with his right non-gloved hand. Inspector spoke with employee who washed hands and put on gloves.

High Priority – Stop Sale issued on time/temperature control for safety food due to temperature abuse

milkshake mix (45-52F – Cold Holding) in milkshake machine next to drive through. Employee indicated machine has been inoperable and they added one bag the day before.

High Priority – Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit

Observed bowl of cooked beef burgers sitting atop unused freezer next to deep fryer at 69F cold holding. Employee indicated been out for 1 hour and placed back in cooler

milkshake mix (45-52F – Cold Holding) in milkshake machine next to drive through. Employee indicated machine has been inoperable for a few days. Corrective Action Taken

High Priority – Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet

Observed no vacuum breaker on either side of splitter on mop sink.
